Chapters need to complete this form - now available online!! - and send it to the Association
Office at the beginning of each fiscal year, after the new year's
officers have been elected by the chapter members.
- Chapter Financial Reporting Form 59k - PDF
Chapter Treasurers need to submit this form to the Association Office twice a year (by October 15th and April 15th) to report income and expenses of the chapter. This information will be used to fill out the IRS Group Tax Return. Please submit the form to:

These two ledger sheets were submitted by a chapter treasurer. We put them into a standard format and hope you find them useful. They are intended to make keeping track of finances easier for chapter treasurers and are not required to be submitted to the Association. The Chapter Financial Reporting Form is required to be submitted twice per year to report on chapter finances.
